Jerusha Hess, the co-writers and director of the film.  Also Jared altered the picture by writing "Sweet" in a quote bubble.  Getting both Jared and Jerusha Hess to autograph the same memorabilia, together with the one-of-a-kind alteration, makes this an extremely rare prize collection item for the biggest Napoleon Dynamite fan!
For an extra fun bit of trivia, I myself purchased this dvd and had this cover personally signed by Jared and Jerusha Hess at the original DVD release party in Salt Lake City Utah in 2004.  I have kept the cover locked away unused in a non-smoking home all these years.
The DVD cover is in pristine Near Mint condition, no tears, creases, stains or markings of any kind (other than the signatures and alter, of course).
I will entertain reasonable best offers for this memorabilia; there appears to be nothing like it for sale on eBay.
